Community Healthcare Center welcomes

Medicare, Medicaid, CHIP, TRICARE and most Insurance.

Sliding fee discount is available based on family size and income, regardless of insurance status.

No-cost Medical Services

In addition to the above listed primary care services, Community Healthcare Center takes advantage of programs which may provide eligible patients with services with a co-pay or possibly no cost. The Sight for Students Program recognizes the importance of vision in the daily social and academic lives of children. Click here for more information for free eyeglasses for children.

Counseling for Children & Adults

The counseling provided by our Behavioral Health Counselors are aimed at helping patients figure out changes they can make in their lives that will improve the issues they are having. Typically, this includes learning new skills to better cope with and manage day-to-day stress, but can also include changing your thoughts to achieve the results patients and their medical professional would like to see. Our counselors work closely with our medical team to ensure that all of the patient’s care team are working together toward common goals. When needed, our counselors will arrange for referral to additional services in the community that may benefit a patient.

  • Depression
  • Anxiety
  • Weight issues
  • ADHD
  • Managing and living with a chronic illness
  • Behavior problems in children
  • Smoking cessation
  • Developing healthy behaviors and habits
  • Stress management
  • Anger management
  • Grief and Loss
  • Coping with chronic pain

Our Behavioral Health Counselor focuses on providing brief interventions and use the following counseling approaches when working with our patients:

  • Brief Solution Focused
  • Cognitive Behavioral
  • Motivational Interviewing

Health Education

  • Illness and disease-related facts
  • Nutrition, diet, and exercise

Outreach Services

  • Screenings and education
  • Eligibility workers to assist patients with local, state, and federal programs
  • Department of Human Services Works Advisor is on-site for assistance with: Medicaid, Food Stamps, and TANF
  • Health Insurance Marketplace Certified Application Counselors are available to offer free and unbiased application assistance and enrollment
  • HHSC Community Partner Program Case Assistance Navigators are available to offer free and unbiased assistance and enrollment with Medicaid, CHIP, TANF, and Healthy Texas Women’s Prorgam
  • Nurse-Family Partnership® is a nurse-led, evidence based program that helps transform the lives of first-time mothers and their children

Screening usually starts at age 45, but can vary based on risk factors.

There are various screening methods available, such as colonoscopy and stool tests.
