Outreach & Enrollment Services
At Community Healthcare Center, our Outreach & Enrollment Department offers free and unbiased assistance and enrollment for a variety of programs, including:
- Medicaid
- CHIP (Children’s Health Insurance Program)
- Health Insurance Marketplace
- SNAP (Supplemental Nutrition Assistance Program)
- TANF (Temporary Assistance for Needy Families)
- Healthy Texas Women’s Program
Our team includes Certified Application Counselors and Case Assistance Navigators ready to support you, whether you are a patient or not. We are available by appointment and by walk-in during regular business hours.
Case Assistance Navigators are trained and certified through HHSC’s Community Partner Program. Our Case Assistance Navigators provide assistance and enrollment with Medicaid, CHIP, SNAP, TANF and Healthy Texas Women’s Program.
Certified Application Counselors are trained and certified through the Centers for Medicare and Medicaid. Our Certified Application Counselors provide assistance and enrollment with Health Insurance Marketplace.
