New Patient Acceptance Policy

Community Healthcare Center (CHC) continues to provide the same high quality and quantity of services to our existing patients; however, we are only accepting new patients within the following vulnerable populations:

  • Ages 20 and under,
  • Ages 65 and older, or
  • Pregnant women.

Persons within the ages of 21 through 64 who are not pregnant and are not already an established CHC patient will need to seek primary care services elsewhere. We will look forward to being able to begin accepting new patients from all age groups and both maternity statuses as soon as possible.

Again, all current patients are welcomed to continue receiving whatever primary care services we offer from us whenever they need it.  Any new patients that we can accept (ages 20 and under, 65 and older, or that are pregnant), will continue to be welcomed too.

COVID-19 vaccines are available for all qualified individuals, patient or not.  Individuals wanting to be vaccinated may schedule their COVID-19 vaccination appointment by calling (940) 766-6306.  However, simply receiving a COVID-19 vaccine from CHC does not establish you as a CHC patient.
