New Patient Appointment Request

You may request a new patient medical, dental or counseling appointment using the form below, or you may call us at (940) 766-6306. Once you have completed the form and submitted it to us you will be contacted by phone no later than the next scheduled business day.

Due to the current situation with Covid-19, and in order to best care for our existing patients and our employees, Community Healthcare Center is only accepting new patients at its Wichita Falls service sites (including TodayCare) within the following vulnerable populations:

  • Under 21 years of age,
  • Ages 65 and older, or
  • Pregnant women.

This form is for new medical, dental or counseling patients only. Established Community Healthcare Center patients must request an appointment by using the patient portal or by calling (940) 766-6306.

Community Healthcare Center (CHC), like other healthcare organizations, has experienced a drastic increase in COVID-19 symptomatic visits from its existing patients and the local community. Due to the related increase in patient volume and decrease in staffing and other resources, CHC has made temporary changes in its patient acceptance policies.

Specifically, CHC will continue to provide the same high quality and quantity of services to our existing patients; however, we will only be accepting new patients within the following vulnerable populations:

• Ages 20 and under,
• Ages 65 and older, or
• Pregnant women.

Persons within the ages of 21 through 64 who are not pregnant and are not already an established CHC patient will need to seek primary care services elsewhere. We will look forward to being able to begin accepting new patients from all age groups and both maternity statuses as soon as possible.

Again, all current patients are welcomed to continue receiving whatever primary care services we offer from us whenever they need it, and regardless of their COVID-19 vaccine status. Any new patients that we can accept (ages 20 and under, 65 and older, or that are pregnant), will continue to be welcomed too, also regardless of their COVID-19 vaccine status.

COVID-19 vaccines are available for all qualified individuals, patient or not, ages 12 and older. Individuals wanting to be vaccinated may schedule their COVID-19 vaccination by calling (940) 766-6306. However, simply receiving a COVID-19 vaccine from CHC does not establish you as a CHC patient.
