📢Attention all Middle School Students, High School Students, Parents, Guardians, and Youth that participate in Youth Sports Leagues!

Community Healthcare Center is thrilled to announce our ninth year to have FREE Sports Physical Clinics available to area students. This year, our first FREE Sports Physical Clinic will be on Thursday, May 2nd from 3pm to 5pm. Don’t miss out on this amazing opportunity to get a head start in preparing to hit the field or court with confidence for the 2024-2025 season!

This event is for all Middle School and High School Students, including those that participate in Youth Sports Leagues. All public school districts are welcome, including private schools and youth sports leagues. Don’t forget to bring that completed sports physical form that is required by your school district or youth sports league signed by your parent or guardian. There is a new U.I.L. sports physical form this year. So, please make sure your sports physical form has the year 2024 in the top right of the front page. Plus, if you wear contacts or glasses, be sure and bring those to the event to pass your eye exam portion of the physical.

🌟The first 250 students to receive a FREE sports physical at our event on May 2nd will receive a FREE "Put Me In Coach" t-shirt.

Spread the word and let’s make sure every student is prepared for the 2024-2025 season ahead! See you on Thursday, May 2nd from 3pm to 5pm at Community Healthcare Center located at 200 MLK Jr. Blvd., Wichita Falls, Texas 76301.

The student does not have to be a patient of Community Healthcare Center to receive a FREE sports physical at this event.

Community Healthcare Center will be hosting a Seasonal Vaccines Clinic on Saturday, April 20th from 8:30 to 10:30am. Vaccines available will be for Flu and Coronavirus.

Quadrivalent Flu Shot may be free or up to $25. Moderna vaccines for Coronavirus is available for those ages 6 months & up.
The vaccine is available for the insured and uninsured. If you have insurance, please bring your insurance card.
