FREE Sports Physical Clinic

on Thursday, June 23rd from 3:00pm to 5:00pm

Community Healthcare Center is having a FREE Sports Physical Clinic for middle school and high school students from 3:00 p.m. to 5:00 p.m. on Thursday, June 23rd at Community Healthcare Center.

This event is for ALL school districts, private schools, and youth sports leagues. All high school and middle school athletes and extracurricular students, such as high school marching band students, can get their sports physical for the 2022-2023 school year completed at this event. The student does not have to be a patient of Community Healthcare Center to participate.

Only the student receiving the sports physical and one parent/guardian should attend the FREE Sports Physical Clinic.

All sports physical forms required by the school district or youth sports league must be completed and signed by the parent/guardian prior to the event. The student must bring the most current sports physical form required by their school district. For U.I.L. participants, the form would have 2020 at the top right of the pre-physical form. The current forms can be found below and downloaded. Due to vision screenings being required for sports physicals, all students that require prescription eyewear must bring their contacts or glasses to the clinic. No sports physical will be done without the completed and signed paperwork by the parent/guardian.

This FREE Sports Physical Clinic is only for students in middle school and high school. Those that participate in youth sports leagues are also welcome to receive a free sports physical at this event. College sports physicals cannot be completed at our FREE Sports Physical Clinics.
