Community Healthcare Center gladly accepts donations. Your contribution allows us to expand and enhance our capacity to care for patients, many of whom would go without care were it not for the Center.

1. Download, print, and fill out this form: Donation Form. Your tax-deductable donation can be sent to the following address:

Please make the check payable to “Community Healthcare Center” and provide your complete name and mailing address so that we may send you a receipt for tax purposes.

We are a 501c(3) nonprofit organization and your donations are tax deductable to the extent the law allows.

There are various screening methods available, such as colonoscopy and stool tests.
