Community Healthcare Center is seeking customer-service oriented, full-time Registered Nurse Home Visitors to join our growing team in the Nurse-Family Partnership® Program. Nurses will utilize both clinical experience and evidence based practice to deliver a strength-based model that emphasizes patient education and empowerment through mutual goal setting and a behavior changing theory in an at-home case management experience. Ideal candidates will possess qualities of both strong work and ethical character. Advocacy for the client and commitment to the program a must. Nurse must be self-directed with the ability to stay on task and complete assignments autonomously. Active participant as a team player is critical. REQUIRED: Registered Nurse, BSN with TX licensure in good standing; CPR; Flexible schedule and attitude as well as ability to travel to trainings. PREFERRED: Minimum 2 years nursing experience in community, maternal or child health, or mental/behavioral health; STRONGLY PREFERRED: Bilingual.
