How do I make an appointment?

You may call us at (940) 766-6306.

We can be reached by phone from 8:00AM to 5:00PM Monday through Friday..

New patients may request an appointment online by clicking here.

Established Community Healthcare Center patients may request an appointment online with their provider through our patient portal.

Do you take walk-ins for medical?

If you would like to see your current medical provider for your medical concerns, we offer convenient same day sick appointments.  Please call (940) 766-6306 to schedule your same day sick appointment.

Community Healthcare Center (CHC) does have TodayCare that accepts same day walk-ins Monday through Friday during normal business hours. TodayCare accepts currently established CHC patients, and welcomes new patients that choose to make CHC their “provider of choice.” Please bring the necessary items below under “What should I bring to my appointment” when coming in for a walk-in appointment with TodayCare.

Community Healthcare Center is not an emergency clinic. If you have a medical emergency, please go to the emergency room at the local hospital or call 911.

Do you take walk-ins for dental?

We do offer acute emergency walk-in hours for dental. If you are having an acute dental emergency, you may come in between 8:30AM and 5:00PM Monday through Friday.

What should I bring to my appointment?

  • Insurance card (if you have health or dental insurance)
  • Picture ID
  • Social Security card
  • Immunization record
  • Proof of household income
  • Bottles of all medications you are currently taking.

What else should I be doing in addition to social distancing and wearing a face covering (face mask)?

You should be frequently washing your hands with soap and warm water for at least 20 seconds at a time. If water and soap is not available, you should be using hand sanitizer that contains at least 60% alcohol or greater.
