How do I make an appointment?

We can be reached by phone during normal business hours Monday through Friday..
New patients may request an appointment online by clicking here.
Established Community Healthcare Center patients may request an appointment online with their provider through our patient portal.
Do you take walk-ins for medical?
If you would like to see your current medical provider for your medical concerns, we offer convenient same day sick appointments. Please call (940) 766-6306 to schedule your same day sick appointment.
Community Healthcare Center (CHC) does have TodayCare that accepts same day walk-ins Monday through Friday during normal business hours, and TodayCare is open on Saturdays from 8:30 a.m. to Noon. TodayCare accepts currently established CHC patients, and welcomes new patients (within our new patient guidelines on our home page) that choose to make CHC their “provider of choice.” Please bring the necessary items below under “What should I bring to my appointment” when coming in for a walk-in appointment with TodayCare.
Community Healthcare Center is not an emergency clinic. If you have a medical emergency, please go to the emergency room at the local hospital or call 911.
Do you take walk-ins for dental?
We do offer acute emergency walk-in hours for dental. If you are having an acute dental emergency, you may come in during normal business hours Monday through Friday.
What should I bring to my appointment?
- Insurance card (if you have health or dental insurance)
- Picture ID
- Social Security card
- Immunization record
- Proof of household income
- Bottles of all medications you are currently taking.
