How do I make an appointment?
Please call us at (940) 766-6306.
Do you take walk-ins?
It is recommended that you make an appointment for medical services, however you may be seen without an appointment. Wait times for walk-ins may exceed those for patients with scheduled appointments.
Walk-in times are:
Medical:
Monday-Friday, 8:00am to 10:00am and 2:00pm to 4:00pm
Dental: Urgent Care Only
Monday-Friday, 8:30am to 9:00am and 1:30pm to 2:00pm
If it is an emergency, please call 911or visit the Emergency Room. Community Healthcare Center is not an emergency care facility.
You must call to schedule an appointment for routine dental services.
What should I bring to my appointment?
Insurance card (if you have health or dental insurance)
Picture ID
Social Security card
Proof of address
Immunization record
Proof of household income
Bottles of all medications you are currently taking.
***Legal guardians must provide proof of guardianship for a minor to receive treatment.
